Please read these Terms of Service carefully before using the Nourishing Life Collective website or scheduling any services. By accessing our website or engaging our services, you agree to be bound by these terms.

1. About Nourishing Life Collective

Nourishing Life Collective ("NLC," "we," "us," or "our") is a whole health practice located in Portland, Oregon. NLC provides integrative health services including naturopathic medicine, acupuncture, East Asian and Chinese medicine, physical therapy, and chiropractic care. Our providers are licensed and/or certified under the applicable laws of the State of Oregon.

2. Acceptance of Terms

By accessing or using this website (the "Site") or by scheduling, receiving, or inquiring about any services offered by NLC, you ("Patient," "User," or "you") agree to be bound by these Terms of Service and our Privacy Policy, which is incorporated herein by reference. If you do not agree to these terms, please do not use this Site or engage our services.

We reserve the right to update these Terms of Service at any time. The most current version will always be posted on this page with an updated effective date. Your continued use of the Site or our services following any changes constitutes your acceptance of the revised terms.

3. No Emergency Services

THE SERVICES PROVIDED BY NOURISHING LIFE COLLECTIVE ARE NOT INTENDED FOR MEDICAL EMERGENCIES. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

NLC does not provide emergency care, urgent care, or crisis intervention services. Nothing on this Site or in communications with NLC providers should be interpreted as guidance for managing a medical emergency.

4. Nature of Services — Not a Substitute for Conventional Medical Care

The services offered at NLC are integrative and complementary in nature. While our providers are licensed professionals in their respective disciplines, naturopathic medicine, acupuncture, East Asian medicine, physical therapy, and chiropractic care are not substitutes for all forms of conventional medical treatment.

Nothing provided through this Site, in consultations, or through our services constitutes:

  • A diagnosis of any medical condition as defined by conventional medicine

  • A guarantee or warranty of treatment outcomes

  • A replacement for care from your primary care physician or specialist when such care is warranted

We encourage patients to inform all of their healthcare providers — including conventional physicians — of all treatments they are receiving, including those provided by NLC.

5. Provider Licensure and Scope of Practice

All NLC providers hold appropriate licensure and/or certification as required by the State of Oregon for their respective disciplines. Services are provided within the applicable scope of practice as defined by Oregon law, including but not limited to:

  • Oregon Revised Statutes Chapter 685 (Naturopathic Medicine)

  • Oregon Revised Statutes Chapter 677 (Medicine)

  • Oregon Revised Statutes Chapter 684 (Chiropractic)

  • Oregon Revised Statutes Chapter 688 (Physical Therapy)

  • Oregon Administrative Rules governing acupuncture and East Asian medicine

Patients are encouraged to ask their provider about their specific credentials, training, and scope of practice at any time.

6. Patient Responsibilities

To receive safe and effective care, patients agree to:

  • Provide accurate, complete, and current health history and medication information

  • Disclose all current treatments, supplements, and healthcare providers

  • Inform NLC providers promptly of any changes in health status or reactions to treatment

  • Follow through on referrals to other providers when recommended

  • Communicate openly and honestly regarding symptoms, concerns, and treatment responses

NLC providers rely on the accuracy of information provided by patients. Providing inaccurate or incomplete information may affect the safety and effectiveness of care and releases NLC and its providers from liability arising from such omissions.

7. Appointments, Cancellations, and No-Show Policy

Scheduling:Appointments may be scheduled online through our booking system, by telephone, or by email. Appointment confirmation does not constitute a guarantee of provider availability in the event of unforeseen circumstances.

Cancellations: We ask that patients provide at least 48 hours’ notice when canceling or rescheduling an appointment. This allows us to offer the time to another patient in need of care.

Late Cancellations and No-Shows: Cancellations with less than 48 hours’ notice, and appointments missed without notice, may be subject to a cancellation or no-show fee of $150. This fee is not billable to insurance and is the patient’s responsibility.

Repeated no-shows or late cancellations may result in discharge from the practice.

8. Fees, Payment, and Insurance

Fees: Current fees for services are available upon request and may be discussed with our administrative team prior to your first appointment. NLC reserves the right to update its fee schedule at any time with reasonable notice to active patients.

Payment: Payment is due at the time of service unless other arrangements have been made in advance. NLC accepts major credit and debit cards. We do not extend credit for unpaid balances without prior written agreement.

Insurance: NLC providers may or may not participate in your insurance plan. It is the patient’s responsibility to verify coverage, obtain referrals if required, and understand their benefits before receiving services. NLC will make reasonable efforts to assist with documentation for insurance reimbursement, but cannot guarantee coverage or reimbursement of any claim.

Amounts not covered by insurance remain the patient’s financial responsibility.

9. Privacy and Health Information (HIPAA)

NLC is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). We are committed to protecting the privacy and security of your protected health information (PHI). Our Notice of Privacy Practices, available separately, describes in detail how we may use and disclose your health information and your rights as a patient.

By using this Site or engaging our services, you acknowledge receipt of our Notice of Privacy Practices.

This Site may use cookies and analytics tools that collect non-identifying information about Site usage. We do not sell or share your personal information with third parties for marketing purposes without your explicit consent.

16. Termination of Services

NLC reserves the right to decline or discontinue services to any patient, at the discretion of the treating provider, consistent with applicable professional and ethical standards and Oregon law. In the event of discharge from the practice, NLC will provide reasonable notice and, where appropriate, referrals to other providers to support continuity of care.
